Derma roller (microneedling) regrows hair

Real adjunct effect — adding microneedling to minoxidil roughly doubles density gain in trials. Standalone effect is modest. 1.5mm roller, once a week, not daily.

The studies, decoded

Dhurat 2013 RCT (100 men, 12 weeks): minoxidil + microneedling 1x/week produced ~91% of patients showing 'much better' improvement vs 21% in minoxidil-only arm. Significant density gains. Standalone microneedling without minoxidil has been studied less and shows weaker effects. The effect is most pronounced as a multiplier on top of minoxidil.

Mechanism

Controlled microtrauma stimulates wound-healing cascade: growth factors (PDGF, VEGF, EGF) drive follicular stem cell activation. Also increases topical absorption of minoxidil through the stratum corneum.

What to actually expect

Realistic outcome

1.5mm roller (not the cheap 0.25mm ones), once a week, applied to the AGA-affected zones, paired with minoxidil. Sterilize between uses. Expect synergy at 4-6 months. Daily use is overuse and not the protocol.

Better alternatives

If you can only pick one: minoxidil alone. Add the roller once you've established the minoxidil routine. Microneedling pens / professional treatments do similar things at higher cost.
